At some point, many employees will experience a disabling illness or injury. Symetra works with claims processing professionals to help insureds get the financial assistance they need while exploring all avenues for returning to good health and work as soon as possible.
To file a claim, insureds need to have the following documentation:
- Employer statement filled out by the group's benefits department
- Employee statement from the insured
- Attending Physician's Statement
- Authorization and Disclosures Form,
signed and dated by the employer
