Symetra is committed to working with you to deliver group life insurance solutions that meet the needs of your clients, their employees and families.

Our underwriting guidelines require a group to be established for at least two years as well as the following information. Details on each will be needed in order for Symetra to release a quote:

  • Class description and benefit schedule
  • Guarantee issue limit and benefit maximum
  • Age reductions for life insurance and AD&D, when applicable
  • A census that includes date of birth and gender. (If benefits are a multiple of salary, salary information is also needed on the census form.)
  • Experience for groups over 500 lives
  • Copy of current contract is required for groups with 500 or more lives, and helpful for all groups

Participation Requirements

Basic Group Life

  • Noncontributory - 100% of eligible employees
  • Contributory - 75% of eligible employees

Supplemental Group Life

  • 15% of eligible employees or 10 lives

Census Template

Groups with 500 or more lives require a complete census, including employees' date of birth and gender prior to quoting. If the benefit is a multiple of salary, then salary information is also needed.

Enrollment and Evidence of Insurability Forms

Your Symetra representative can help facilitate the enrollment process and work with you and the beneficiary in the event of a claim.

Conversion Forms

The Conversion Right enables an eligible employee, whose coverage under a Symetra group life policy is terminating, to request to continue all, or part, of their life Basic or Supplemental insurance coverage. Benefits may continue under an individual conversion policy and employees do not have to provide Evidence of Insurability.

Portability Forms

The Portability Continuation Privilege allows a terminated employee to continue the coverage that was in-force under the Symetra Supplemental Group Life policy at group rates as long as the portability option is included in the policy.
